The Affordable Care Act, rising expenses of Medicare, and the increasing cost of healthcare to employers and employees have led to renewed efforts to control health expenditures.  Most care models are organized for treatment of acute illness, while chronic disease treatment is often continuous rather than a short-term event.

Numerof’s Dr. David Horwitz covers four elements of implementing a chronic care model in his article, Implementing the Chronic Care Model: A Grassroots Approach, published in Physician Leadership Journal.  David highlights the urgency for more efficient and cost-effective patient management, and how a chronic care model can improve the healthcare experience for patients and providers alike.
