Numerof Consultant Gordon Phillips spoke to members of the California Hospital Association in Huntington Beach, California on creating high value partnerships across the continuum of care.  With the rise of value-based payment models, providers are increasingly responsible for factors beyond their immediate control, and building relationships with trusted partners is crucial for minimizing risk and achieving cost and quality goals. Gordon outlined why and how organizations are moving to value, and walked through the stages of establishing strategic partnerships with SNFs and other organizations.
