CMS is right to target wasteful spending in Medicare, but there has been much debate surrounding the new WISeR (Wasteful and Inappropriate Service Reduction) Model, and that it takes the wrong approach. By forcing prior authorization (PA) into traditional Medicare and outsourcing reviews to contractors incentivized by claim denials, WISeR threatens to replicate Medicare Advantage’s worst administrative headaches. AI shouldn’t be used as a barrier to care. Instead, CMS should leverage data-rich medical records to provide physicians with peer-benchmarked feedback on costs and outcomes. True progress comes from establishing physician accountability, not adding bureaucratic red tape that delays needed patient care.
